USS IWO JIMA (LPH 2), CIWS Misfire, 11 October 1989    

 

On October 30, 1990, the amphibious assault ship, USS Iwo Jima (LPH 2) experienced a boiler casualty while getting underway from Manama, Bahrain. A leaking boiler valve blew causing large amounts of steam to be dumped into the boiler room. There were 10 casualties reported as a result of the explosion.

On October 11, 1989, the Charleston class amphibious cargo ship, USS El Paso (LKA 177) accidentally fired CIWS rounds into the amphibious assault ship USS Iwo Jima (LPH 2) while conducting gunnery exercises. There was one personnel casualty and one injury reported as a result of the misfire.

USS IWO JIMA (LPH 2), Fire, 3 July 1979

On July 3, 1979, the amphibious assault ship, USS Iwo Jima (LPH 2) experienced a fire in two berthing spaces while in port Norfolk Naval Shipyard. There were no personnel casualties but five injuries reported as a result of the fire. A crewmember was arrested 3 days later for arson.

USS IWO JIMA (LPH 2) and USS NASHVILLE (LPD 13), Collision, 3 March 1975

On March 3, 1975, the amphibious assault ship, USS Iwo Jima (LPH 2) collided with the Austin class amphibious transport dock USS Nashville (LPD 13) when the assault ship lost steering control during a highline transfer southwest of the Azores. There were no personnel casualties or injuries reported but there was severe damage to both ships as a result.

USS CORAL SEA (CV 43) and USS IWO JIMA (LPH 2), Collision, May 21, 1966

On May 21, 1966, while transiting in port to San Diego, CA, the Midway class aircraft carrier USS Coral Sea (CV 43) bumped the amphibious assault ship USS Iwo Jima (LPH 2), which was anchored out in the harbor. There were no personnel casualties or injuries reported.
